Functional Capacity Assessments
A Functional Capacity Assessment (FCA) evaluates your ability to perform daily tasks at home, work, or in the community. The assessment helps identify your goals, support needs and opportunities for capacity building.
A FCA is often needed if you are applying to the NDIS, new to the NDIS or have had significant changes in your circumstances.
Home
Modifications
Modifications to your home to improve safety and facilitate daily tasks. These changes can range from minor adjustments, like installing a grab rail, to more complex modifications, such as creating a level-entry bathroom. Home modifications funded by the NDIS, TAC, or a Home Care Package must meet specific approval criteria.
Postural, Positioning and Pressure Care Supports
Guidance on equipment or devices that can help you maintain proper body alignment, posture, and pressure care, especially when sitting or lying down. These supports are particularly beneficial for individuals who spend extended periods in wheelchairs or beds. Common types of supports include adjustable wheelchairs, beds, and commodes, wheelchair cushions, pressure-relieving mattresses, backrests, lateral supports, pelvic positioning belts, headrests, and bed pillows and wedges.
Vehicle
Modifications
Guidance on vehicle modifications and equipment to assist with entering, exiting, and traveling in your vehicle, as well as loading and unloading mobility equipment or other necessary items.
Cognitive Assessment and Intervention
Therapeutic support to identify and address the impact of cognitive impairment on daily activities. This support is tailored to your individual circumstances and goals. It may include conducting standardized and function-based assessments, developing personalized intervention plans, or making environmental modifications and prescribing aids to enhance cognitive functioning.
Home and Living Assessments
A home and living assessment examines your needs, circumstances, and goals concerning your living environment. The NDIS provides several housing and support funding options for eligible individuals. These options include Specialist Disability Accommodation (SDA), Supported Independent Living (SIL), and Individualised Living Options (ILO).
Assistive Technology (AT)
Guidance on equipment or devices that can help you perform tasks that are challenging due to disability or age-related changes. Common types of assistive technology include wheelchairs, scooters, hoists, powered beds and recliner chairs, pressure-relieving mattresses, walking frames, and kitchen and dressing aids.
Capacity
Building
Therapeutic support to help you develop the skills, knowledge, and resources needed to perform daily tasks more safely and independently, and to achieve your goals. This support is tailored to your individual circumstances and objectives. It may involve teaching adaptive techniques using aids or modified routines to make self-care and cooking activities easier, developing strategies and systems to better organize your household, remember important tasks, manage your routines, or help you feel confident navigating the community more independently.
Lymphoedema
Clinic
Lymphoedema assessment and management provided by a qualified Lymphoedema practitioner. This includes lymphatic drainage massage, compression therapy, skin care, and self-management education.
Home and Functional Assessment for Older Adults
An assessment to identify any challenges you may encounter with daily tasks and moving around your home. A report will then be provided, outlining recommendations to make things easier and safer for you. These recommendations could include equipment, home modifications, or in-home supports.
Upper Limb
Therapy
Therapeutic support to help you manage spasticity and regain strength, coordination, and function in your arms and hands. This includes splinting, serial casting, prescribing silver ring splints and braces, functional electrical stimulation, task-specific training, constraint-induced movement therapy, and mirror therapy.
Relationships, Sex
and Intimacy
Therapeutic support to help you develop the skills needed to successfully and safely navigate and participate in relationships, sex, and intimate activities. This support is tailored to your individual circumstances and goals.
It may include education on sexual health, assistance in developing communication techniques to express needs and desires with partners, and providing positioning strategies or adaptive equipment to ensure comfortable and safe sexual activity.
Manual Handling Training and Plans
Guidance to ensure that those assisting you with movements and transfers are well-trained and knowledgeable about your specific needs. This process aims to minimize the risk of injury to both you and your caregivers, while also enhancing the overall quality of care provided.
